Epitalon is sold on a single word, telomeres, and a single claim, that a Russian trial showed it extends human life. The telomere result is real and comes from a dish. The trial is real and used a different product. Working out which studies are about which molecule is most of the job on this page.
What it is
Epitalon (also epithalon, epithalone; sequence Ala-Glu-Asp-Gly, AEDG) is a synthetic tetrapeptide designed in the 1990s by Vladimir Khavinson's group at the St Petersburg Institute of Bioregulation and Gerontology to reproduce the activity of epithalamin, a peptide extract of bovine pineal gland the same group had studied since the 1970s. Proposed mechanisms include telomerase activation, melatonin regulation and epigenetic effects on gene expression (a 2020 paper on neurogenesis, and the 2025 review PubMed 40141333, summarise them).
The evidence
Cells. Epitalon induced telomerase activity and telomere elongation in cultured human somatic cells in 2003 (PubMed 12937682). An independent group reproduced telomere lengthening in human cell lines in 2025 (PubMed 40908429), attributing it to telomerase upregulation or alternative lengthening. This is the strongest evidence for epitalon, and it is in vitro.
Humans, epitalon itself. One study: a 2002 controlled clinical report in retinitis pigmentosa (PubMed 12195242), alongside a rat model, from the developer's institute, describing a positive clinical effect in 90% of treated patients. Small, unblinded as far as the abstract reports, unreplicated.
Humans, epithalamin (the extract). The 2003 paper titled "Peptides of pineal gland and thymus prolong human life" (PubMed 14523363) reported that older adults given pineal and thymus peptide preparations over several years had lower rates of respiratory illness, heart disease and other conditions and lower mortality than controls. Related papers from Korkushko and colleagues in 2006 and 2011 report long-term follow-up. These are the studies that appear under "epitalon" on most sales pages. They are studies of epithalamin, a multi-peptide gland extract. Epitalon was designed to mimic it; whether it does in people has not been tested.
Regulatory status
FDA placed epitalon in Category 2 on September 29, 2023. The Category 2 page (content current as of April 22, 2026) shows it as nominated but withdrawn, with the concern retained: possible immunogenicity from aggregation and impurities, and "FDA has not identified safety-related information regarding epitalon for the proposed route of administration."
Epitalon is on the July 24, 2026 agenda of the Pharmacy Compounding Advisory Committee. It is not on the 503A bulks list, not in Category 1, and not an ingredient of any FDA-approved drug. See the 503A explainer.
Safety signals
No human safety data for the route sold. A compound whose selling point is telomerase activation raises an obvious question, since telomerase activity is a feature of most cancers; no human study has examined it, and the cell-line papers do not address it. That is not evidence of harm. It is the absence of the study a regulator would ask for first.
Grade: C + X, lowest confidence
C on the letter of the scale, because one controlled human study of epitalon exists. We attach the lowest confidence in the tier: single-source, small, unreplicated, and in an indication (retinal degeneration) unrelated to the longevity claim. The epithalamin trials are excluded from the grade because they tested a different product. X for FDA's published safety concern. Questions people ask
Does epitalon lengthen telomeres in humans?
It has been shown to induce telomerase activity and lengthen telomeres in cultured human cells (PubMed 12937682, and again in 2025, PubMed 40908429). No published study has measured telomere length in people before and after taking it. A cell-culture result is the starting point for a human study, not a substitute for one.
Didn't a Russian trial show epitalon extends life?
The trial usually cited (PubMed 14523363, 2003) reported lower illness rates and mortality in older adults given pineal and thymus peptide preparations over years. The pineal preparation was epithalamin, an extract of bovine pineal gland containing many peptides. Epitalon is the synthetic four-amino-acid peptide later designed to mimic it. The two are related and are not the same product, and the trial's methods have not been independently replicated.
What human study of epitalon itself exists?
A 2002 controlled clinical study in retinitis pigmentosa from the developer's institute (PubMed 12195242) reported a positive clinical effect in most treated patients. It is small, single-source and unreplicated. We found no other interventional human study of the tetrapeptide.
